ADDITIONAL SERVICES THAT ARE OFFERED IN OUR OFFICE: All nutritional services are cash services. We will not bill your insurance. Please be prepared to pay your bill at the time of service. Hair Analysis – Payment must be made on the day that we collect your hair sample. 5. Nutritional Supplements • Our office carries several complete lines of supplements. • Protocols are individual to each patient. Dr. Annette will provide you with a recommended protocol following your nutritional consultation and lab results. 6. Ionic Cleansing • $35.00 for a half hour session • $300.00 for ten half hour sessions • After completing a series of ten sessions, you can start a monthly plan consisting of 2 cleanses per month for $55.00 IF YOU ARE EVER INTERESTED IN ANY OF THE ABOVE MENTIONED SERVICES, PLEASE LET JESSICA KNOW AND SHE WILL GLADLY GET YOU MORE INFORMATION OR SET YOU UP AN APPOINTMENT. ALL PRICES SUBJECT TO CHANGE. Supplement Return Policy: Supplements must be in their original box/package. Boxes and bottles must be unopened. No expired products will be accepted. Any returns, for any reason are subject to a 20% restocking fee. No exceptions. Note: We will give you an additional 5% off the purchase of your supplements if you pay by cash or check.
