In-office Rates:

  • Initial Consultation - 60 minutes - $120.00
  • F/U Consultation without test results - 30 minutes - $50.00
  • F/U Consultation with 1-2 test results - 45 minutes - $70.00
  • F/U Consultation with 3 test results - 60 minutes - $90.00
  • Shape Reclaimed Diet Initial Consultation - 45 minutes - $65.00
  • Shape Reclaimed Diet F/U Consultation - 30 minutes - $50.00
  • Additional fee if consultation goes over allotted time - $25.00 per 15 minutes


Patient Phone Consultation Rates:

  • Initial Consultation - 60 minutes - $50.00 flat rate plus $1.50 per minute
  • F/U Consult w/ test results - approx. 45 min. - $50.00 flat rate plus $1.50 per minute
  • F/U Consult without test results - approx. 15-30 minutes - $1.50 per minute
